I hearby authorize Orillia Minor Hockey Association to conduct any investigation deemed necessary to verify my credentials and qualifications.
I further agree to abide by the Association's Code of Conduct, Policies and Bylaws and the Association's Coaches Code of Conduct.

I agree to obtain & upload an approved Vulnerable Sector Check as per the OHF guidelines and I understand I may be removed as a coach/team official if the vulnerable sector check is not received by September 1st, 2025.
