As you know, all Coaching Staff, Trainers, Managers, Board Members, On Ice Volunteers and "At Large Volunteers are required under OMHA rules to be screened.  This is being done with a Vulnerable Sector Screening (VSS).  The Vulnerable Sector Screening will be valid for 4 years but your are required to sign a Declaration for the 3 years after your Vulnerable Sector Screening has been submitted.

There is no cost to either you or the Orillia Minor Hockey Association to have this VSS done but it is your responsibility to have it done and submitted to Cathy O’Connor.  Here is the procedure for obtaining a Vulnerable Sector Screening:

  1. Print of a copy of the Vulnerable Sector Check Form and Vulnerable Sector Letter (OPP) Copy):....Vulnerable_Sector_Check_Letter_2018-2019_good_copy_(004).pdf and Police_Check_-_Vulnerable_Sector_Form.pdf
  2. Fill in the all the required information.
  3. Print off the signed letter and fill in the date and your name. 
  4. Take the completed request to the Orillia OPP Detachment located at 66 Peter St. Orillia along with 2 pieces of identification.  Click here to see a list of accepted identification.
  5. Once at the Detachment, you will turn over your completed Vulnerable Sector Check Form and the signed letter to the staff and provide two pieces of accepted ID.    
  6. Please ask for a receipt that you submitted your Vulnerable Sector Check Form with the date you submitted it and turn it into the hockey office at Rotary Place. You must ask the staff at the local detachment for the receipt, they will not ask you if you need one.  There is a box outside the hockey office to submit documents. 
  7. After all the paperwork is completed they will let you know approximately how long it will take to get your VSS back and you will have to go back and pick it up.
  8. PLEASE NOTE, that if your name and birth date are similar to someone who has a conviction, you may be asked to submit finger prints before they can provide the VSS.
  9. Once you have your Vulnerable Sector Check Form from the OPP please go to the section below.

If you have confirmed with Cathy O’Connor and we have a copy of the Vulnerable Sector Check Form that some of the Orillia coaches submitted last few seasons, you are required to complete a Declaration (Orillia Minor Hockey).  Here is the procedure:

  1. Complete the following Police Check - Orillia Minor Hockey Declaration Form Police_Check_-_Declaration_Form_Orillia_Minor_Hockey_.pdf 
  2. Complete it and then submit it to the Orillia Minor Hockey office immediately. This declaration will give you the authority to go on the bench.

READY TO SUBMIT YOUR PAPERWORK:

Once you have either your new VSS please submit this to the Orillia Minor Hockey Office immediately, in a sealed envelope with "Cathy O’Connor" written on the front.
