I further agree to abide by the Association's Code of Conduct, Policies and Bylaws and the Association's Coaches Code of Conduct.

I agree to obtain & upload an approved Vulnerable Sector Check as per the OHF guidelines and I understand I may be removed as a coach/team official if the vulnerable sector check and/or declaration is not received by September 1st, 2026.
