I hereby authorize Orillia Minor Hockey Association (OMHA) to conduct any investigation deemed necessary to verify my credentials and qualifications.  I further agree to abide by the Association's Code of Conduct, Policies and Bylaws and the Association's Coaches Code of Conduct.

I agree to provide a vunlnerable sector check to OMHA and I understand that I may be removed as a coach/team official if the vulnerable sector check is not received in the hockey office by September 1st of the current hockey season.
